The Transitions program provides young adults with disabilities in their 20’s and 30’s with supports that enable them to live and thrive in the community.
The program offers services tailored to the young adult age group:
- Safety services
The program encourages opportunities to form meaningful relationships, personal growth and life skills development.
Participants live in their own apartment suites in an apartment building on Cleveland’s east side. They may receive daily supports from other JFSA programs as needed, such as a home health aide to help with daily tasks of independent living (cooking, errands, cleaning, etc.) A JFSA staff member provides daily support and leads group activities, such as social and recreational outings.
Services are funded by a combination of Individual Options Waiver, Level One Waiver, and private pay.
