Nursing
Registered nurse
develops a care plan with the patient and attending physician to ensure
that all aspects of medical care are provided. The nurse provides
assessment, treatment and instruction.
Physical
Therapy
A physical therapist
evaluates the patients level of function in conjunction with the
physician and helps to restore their function to the optimum level. A PT
provides evaluation of their ability to function in their activities of
daily living, along with therapeutic exercise and instruction on a home
exercise program.
Occupational
Therapy
An occupational
therapist works in conjunction with a physician in evaluating activities
of daily living. They retrain and restore an individual's functioning to
the optimum level. They provide adaptive devices, such as eating
utensil, dressing devices and personal hygiene devices.
Medical Social
Workers (MSW)
A medical social
worker provides counseling and access to community agencies and support
groups.
Home Health
Aides
Individuals who have had strokes, heart problems, serious surgeries or are
recovering from a serious illness or are receiving intensive rehabilitation
can receive the following services from our highly trained Health Care Aides:
- Preparing a special
diet and assisting with feeding
- Monitoring and reporting
skin conditions
- Monitoring food and
fluid intake
- Reminding clients of
medication schedule
- Assisting with exercises
and rehabilitation activities
- Empty and care for
a catheter or ostomy bag
- Assist with special
devices, such as canes, walkers, wheelchairs, artificial limbs
- Assist with simple
procedures as directed by the plan of care
- Identify and report signs and symptoms needing attention
- Assisting with bathing,
dressing, hair care and grooming
- Assisting with toileting
and monitoring elimination
- Assisting with ambulation
and transfers
- Assisting with exercises
and rehabilitation activities
