The Care Management Plan
The basis of all Community Care At Home services is the Care
Management Plan. This plan provides you and your family with all the details
of recommended care and responsibilities of both the client and the home health
aide. A Care Management Plan provides clients and their families with a framework
and "peace of mind" knowing that the best care is being maintained.
The first step in developing a personal care plan is a visit
from a Registered Nurse who meets with a prospective client at their home
(family members are welcome) and performs an assessment of individual needs.
There is no charge for this visit. Based on the needs identified during the
home visit, an individual Care Management Plan is developed and serves as
a guide to implement and supervise care on an ongoing manner.
In addition to the Care Management Plan, Community Care at Home
services feature:
- Ongoing supervisory home visits by RNs
- Maintenance and modifications of Care Management Plan as needed
- Coordination of care needs with other medical providers and
family members as appropriate
- Household management including assistance with groceries, home maintenance,
cleaning
Personal Care
Aide/Homemaker/Companion
Individuals who are recovering from a minor illness, or those that require
assistance to remain safely at home can receive the following types
of services from our personal care aide, homemaker or companion:
- Meal planning, preparation and shopping
- Transportation and
escort services
- Laundry and light housekeeping
(this includes vacuuming, dusting, making beds)
- Observing, monitoring
and reporting a client's condition
- Providing activities
to help increase the client's independence
- Maintaining a safe
and secure environment
- Assisting with bathing,
dressing, hair care and grooming
- Assisting with toileting
and monitoring elimination
- Assisting with ambulation
and transfers
- Assisting with exercises
and rehabilitation activities
