Services

 

JFSA's Ascentia is dedicated to providing community-based, supportive services to adults with serious mental illness and their families. JFSA Ascentia clients usually have a diagnosis of schizophrenia, bi-polar disorder or other serious mental illness. JFSA's Ascentia offers the following services based on diagnosis, functioning and individual needs:

  • Community Psychiatric Support Treatment (CPST)

  • Intensive Case Management (ICM)

  • Cognitive Enhancement Therapy (CET)

  • Pharmacologic Management Service

  • Vocational and Employment Services

  • Socialization activities (Social Action Team)

  • JFSA community residence

 

The vast majority of services are provided in the community or within the home of the individual.

 


 

Eligibility

 

Payment sources include Medicaid, Medicare, private insurance and/or private pay.

Community Psychiatric Support Treatment (CPST)

 

Our main job is to help individuals in living as independently in the community as possible. This means helping them to accomplish the "normal" activities of living, such as paying bills, doing chores, getting to the doctor, exercising or looking for a job. Most services are provided in community settings where problems may occur and support is needed rather than in staff offices or clinics.

Intensive Case Management (ICM)

 

For clients with an acute and chronic need for care, Intensive Case Management is available to provide more frequent contact. Case loads are smaller so staff are able to do more intensive work.

Social Action Team

 

This peer-lead group provides social engagement and volunteer opportunities for consumers that want to connect with others with similar issues and to be out and about in the community. Our goal is to provide natural peer-to-peer support which will enhance independence, self-esteem and confidence. The increase in the skills often leads to increased ability to obtain competitive employment. Emphasis is on giving back to the community through volunteer activities such as the Cleveland Food Bank, community gardening, NAMI walk and working at an animal shelter. Pot-luck, movies and Jazz in the Park are just some of the social outings they have done.
